1. Over the Phone

Call 1-800-821-7513 Seven days a week from 8 am to 8 pm.

2. Online

Enroll in a Bravo Health plan online:

• Plans with prescription drug coverage
• Plans without prescription drug coverage

Medicare beneficiaries may enroll in Bravo Health through the Centers for Medicare & Medicaid Services Online Enrollment Center, located at http://www.medicare.gov/ or call 1-800-MEDICARE (1-800-633-4227), 24 hours a day/7days a week. TTY/TDD users should call 1-800-325-0778.

3. Through the Mail
